Before making any changes to the exercise, diet, nutritional, vitamin, mineral or botanical supplementation of the undersigned, a licensed physician should be consulted.

Susanne Wright does not diagnose, cure or treat any illness or disease, rather provides education and support in a health coaching capacity. It is understood and agreed that diagnosing or identifying any medical condition or disease is not part of natural health services provided by Susanne Wright.

This information is not intended to, cannot, and should not be expected to substitute for a personal consultation with your own physician. Further, the undersigned releases Susanne Wright, their lab partners, independent representatives, associates and affiliates from any and all liability for any failure to identify any medical condition or disease. However, the personal physician of the Client may use these same laboratory results to diagnose and/or treat disease.

GENERAL REPRESENTATION AND WARRANTY
